Appointments
Please call our office (972) 899-2271 to schedule an appointment during normal business hours (M, W-F 8am-4pm). Our hours are Mondays, Wednesdays- Friday 8 am- 4 pm. We are closed on Tuesdays. If you call when the clinic is closed, kindly leave a message and we will get back to you as soon as possible. We are closed on legal holidays and weekends, but if you leave a message we will return your call the next working day to schedule an appointment.
When making an appointment, you will be asked to provide the medical reason for your appointment, your insurance information, and the names of your primary care physician and/or referring physician. Please have this information available.
Cancellations
If you need to change or cancel your appointment, our office requires 24 hours notice for all changes in appointment. Please note you will be charged for a missed appointment. As a courtesy, our office will call you in advance to remind you of your appointment.
Prior to your appointment
Please plan to arrive 15 minutes before your given appointment time if you are a new patient, and 10 minutes before your appointment time if you returning for a follow up visit. Please bring your insurance card, personal identification (driver's license), co-payment (we accept cash, checks, Visa/ MasterCard, and debit cards), deductible (if needed), previous medical records/ labs/ imaging, glucometer readings / glucose logbook/ CGM/ Insulin pump (if diabetic), and forms (for new patients). Please have your labs done at least one week before your visit if possible so you can review them with Dr. Raj.
Please have your labs/ imaging/ medical records faxed to our office( fax #) in advance if possible. Dr. Raj can then review them in advance of your appointment.
